The Field-Based Remote Behavioral Health Case Worker plays a critical role in managing and coordinating care for Members with behavioral health conditions and complex needs.
Main Responsibilities
- Develops a plan of care through shared decision-making with the Member/caregiver and in collaboration with providers and other care team members.
- Reviews medication lists and coordinates pharmacy needs with community pharmacists.
- Establishes points of contact with community, medical, and behavioral health teams.
- Maintains accurate documentation of Member interactions in electronic care management platforms.
- Monitors utilization and coordinates services with other payer sources.
- Develops knowledge of ACFC electronic care management platforms, care management programs, and regulatory requirements.
- Conducts face-to-face visits with Members at their residences or community locations.
Requirements
- Bachelor's Degree in Human Services or related field.
- Proficiency in MS Office, internet applications, and electronic medical record and documentation programs.
- Ability to work remotely and travel to access Members' homes and community locations within assigned regions.
