AmeriHealth Caritas is seeking a skilled Care Manager II to join our team. As a Care Manager II, you will be responsible for managing and coordinating care services for members with acute, chronic, medically complex, and/or behavioral health conditions.
The ideal candidate will possess strong clinical knowledge, critical thinking, and communication skills. You will work closely with the care team to develop and implement effective care plans that promote optimal health outcomes.
Key responsibilities include:
- Assessing member situations and developing personalized care plans
- Coordinating care services and referrals with community, medical, and/or behavioral health teams
- Monitoring member progress and making adjustments to care plans as needed
Requirements for the role include:
- Local residence in Jacksonville, NC, to engage with members via face-to-face visits
- Current, unrestricted North Carolina RN license or Master's degree in social work
- Associate degree in nursing required, Bachelor's degree preferred
- Minimum 3 years of professional clinical practice experience as a RN or LICSW working with adults with complex medical and behavioral health diagnoses
