As a Care Manager II at AmeriHealth Caritas, you will play a critical role in achieving this vision by providing high-quality care coordination services to our Members.
You will work closely with healthcare providers, community organizations, and social services to ensure that Members receive seamless and effective care.
Requirements:
- Associate's Degree in Nursing required; Bachelor's Degree preferred.
- Current and unrestricted Michigan RN licensure.
- Minimum of 3 years clinical experience as a RN caring for patients with chronic comorbidities (e.g. asthma, diabetes, sickle cell, hypertension).
- Preferred practice settings include acute care, ambulatory care, home health and community health.
