- Develop and implement care plans that address the physical, emotional, and social needs of Members.
- Collaborate with healthcare providers, community organizations, and social services to ensure that Members receive seamless and effective care.
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
