Amerihealth Caritas is seeking a dedicated Patient Advocate Navigator to join our team. In this role, you will establish connections with members through home visits and outreach calls, identifying social determinants of health needs and addressing them effectively.
You will also exercise judgment and discretion in the coordination of care, collaborating with care management teams, community-based healthcare providers, and other organizations. This position requires strong communication skills, with the ability to inform members of required actions to fulfill HEDIS, EPSDT, and Plan requirements.
