AmeriHealth Caritas is seeking a talented and passionate individual to join our team as a Field-Based Remote Behavioral Health Case Worker. In this role, you will have the opportunity to make a meaningful difference in the lives of our members by providing comprehensive care coordination and support.
Responsibilities:
- Assess member situations, develop care plans, and teach self-management skills to promote optimal health outcomes.
- Collaborate with community, medical, and/or behavioral health teams to establish a comprehensive network of care and support.
- Ensure timely and accurate documentation of member interactions in ACFC electronic care management platforms.
- Identify and escalate quality of care issues, making recommendations to improve member outcomes.
- Develop and maintain a working knowledge of ACFC electronic care management platforms, care management programs, and regulatory requirements.
- Participate in care management rounds to present cases and receive feedback from colleagues.
- Support a positive workplace environment, sharing clinical knowledge and skills to enhance member care.
- Conduct face-to-face visits at member residences or community locations to provide education and support.
- Other duties as assigned.
