Job Summary AMERIMED-MIH Case Manager is seeking experienced, highly motivated individuals to fill the position of Case Manger (CM) The CM is responsible to work with partner health systems and referral source care coordination teams to identify, interact with, and schedule patients who would benefit from our Mobile Integrated Healthcare services.

Requirements:
- Healthcare experience includes at least 3 years of clinic or hospital bedside experience. One year of case management or care coordination experience is preferred.
- Ability to utilize marketing/sales techniques to identify and assist in generating new business in assigned markets.
- This position requires daily travel to referral sources. Reliable access to transportation and a valid Driver’s license, with clean driving record is required.
- This is a full-time position and must be 21 years of age. Background check and passing a drug screen are required pre-employment
- Must have working knowledge of Microsoft Office and comfortable learning new computer platforms/electronic medical records.
- High school diploma or equivalent. Current LPN or RN nursing license in the state of Georgia required. Bachelor’s degree in healthcare or business-related field preferred.
Responsibilities Include:
- Develop, cultivate relationships and act as in-person liaison with all referral sources in the market by providing regular on-site visits and in-person communication with case managers, transition care teams, and others. Referral sources include, but are not limited to hospitals, LTACHs, home care, case managers, etc.
- Assist the Business Development team in identifying new business opportunities within the market.
- Coordination with care team members at partner health systems within specific markets
- Assess and identify social, emotional, financial, personal, and medical needs to ensure patient is appropriate for discharge to Amerimed Mobile Integrated Healthcare. Coordinate with referring providers and care coordinators to ensure appropriate discharge is achieved. Communicate with and involve patient/significant others in discharge planning process.
- Maintain current knowledge of insurance requirements and guidelines.
- Communicate and collaborate effectively with patients, family members, case managers, healthcare providers, and co-workers, including educating patients and family members regarding Amerimed Mobile Integrated Healthcare services
- Provide daily documentation of onsite visits and communications with patients, referral sources, and other market activity.
- Schedule initial visits with the patient. Communicate accurately with the Amerimed MIH scheduling team.
- Team player, able to work effectively and efficiently under pressure and within time constraints.
- Follow-up with patients after in-facility visit
- Other duties as assigned

Pay: $22.50 - $26.89 per hour
