Responsibilities:
Under the supervision of the Director, Medical Management, the Supervisor, Integrated Case Management (ICM), is responsible for the daily oversight of the Case Management unit providing clinical direction, organization, and monitoring of all case management services. Responsible for direct supervisory oversight of staff conducting case management screening, care coordination, case management and other supportive functions. Assists in carrying out the plans and daily operations of the Case Management unit. Provides unit and departmental leadership.
Education/Experience:
- RN: Current, unrestricted (MI) RN license in good standing. Bachelor’s degree required.
- LMSW or LCSW: Master’s degree in social work and current, unrestricted (MI) LMSW/LCSW license in good standing.
- Certified as a Case Manager, preferred
- 1-3 years field supervision
- 3-5 years case management
- May require some field work with care managers and/or community health navigators.
- Proficiency with Microsoft Office Suite (Word, Excel, Power Point). Access is a plus.
