JOB DESCRIPTION:
The Client Coordinator I is the staff responsible for the supervision of AMFM Healthcare’s clients participating in the program. The CCI helps to provide a safe, supportive, and caring environment to promote well-being, safety, and encourages clients to continue their recovery program. This position is expected to cooperate as a team member with AMFM Healthcare management and clinical staff. The CC I has knowledge, skills, and experience with supervising client services and all related tasks to provide a safe, comfortable, and caring environment for clients, and is familiar with concepts of recovery from mental health and co-occurring disorders.
QUALIFICATIONS:
Must be 21 years or older to apply for this position.
Successful completion of the ADP Selective Screening Services background check as well as any other state, federal or company required background/record check.
Minimum Educational Requirement: High School Diploma or GED
Possess valid driver’s license with ability to be insured by our auto insurance policy which includes, but not limited to, the following requirements: 4 continuous years of driving experience, 21 years of age or older, less than 3 moving violations in the last 3 years, and no more than 1 major violation in the last 3 years.
CPR/First Aid Certification
Current T.B. clearance
Physical Requirements: Ability to work on a computer/keyboard, use electronic systems, communicate with others through written and verbal formats, ability to sit, stand, or walk for long periods of time, ability to climb stairs, listening, reading comprehension, lifting up to 20 lbs.

DUTIES AND RESPONSIBILITIES:
The Client Coordinator I will be assigned projects, based on his/her level of expertise, that include any, or all the following job responsibilities.
Coordinates and transports clients safely to all daily activities, ensuring the clients arrive on time to all scheduled activities, and ensures participant safety.
Utilizes intervention and communication skills in working with clients, their families and significant others daily and in crisis.
Rigorously adheres to all Joint Commission policies and procedures, including Waived Testing, Infection Prevention and Control, as well as Emergency Management and Environment of Care procedures.
Proficiently navigate and utilize the BestNotes electronic health record system to document client activities effectively
Collaborate with the program nurse and clinical team regarding medication management, and monitor and report client behavior with medication side effects. Monitor and document client self-administration medications while coaching clients in self-management and documenting medications in accordance with the organization’s policy on medication use.
Maintain the integrity of the residences, ensuring optimal cleanliness at all times and keeping the area hygienic. Conduct orientation to new residents, including reviewing safety drills and locations of fire extinguishers, emergency evacuation diagrams, exit routes, evacuation procedures, and first-aid kits
Facilitate recovery and clinically indicated activities and groups according to organization policy and document into the electronic health record.
Conduct observation rounds, as needed to ensure client safety, and document client behavior and participation in treatment. Effectively and correctly report these in daily passdowns. Responsible for client attendance of daily recovery activities as scheduled.

- Waived Testing
- Able to accurately collect POC samples from clients under observation
- Accurately file information into the clinical record
- Communicate results of Waived Tests effectively through our email communication system
- Medication Management
- Accurately monitor the self administration of medications by clients
- Ensure the integrity of the medication self administration process by appropriately documenting into the eMARS systems with our electronic health record.
- Monitor and record client vital signs
- Communicate discrepancies with nursing
- Correctly provide inventory information to nursing and complete the medication management process with minimal to no errors.
- Group Facilitation and Documentation
- Properly facilitate basic client service groups by engaging clients in the group, monitoring their behavior, and guiding them through the activity/session.
- Submit required group notes in a timely manner
- Have the correct information submitted on all clinical documentation
- Accurately record and state the group topic, evidence based practices utilized in the session, and summarize the overall group participating conclusively
- Document each client's behavior during the group, state the quality of their participation.
- Record a daily passdown in BestNotes and via the organizationals email service that contains an accurate portrayal of each clients disposition, the state of the house, and other pertinent items of note.
- Standards Compliance: EOC, IC, Safety, EM
- Successfully pass routing physical inspections of the cleanliness of the residence under your care and supervision.
- Submit maintenance requests once deficiencies outside the scope of general cleaning are identified.
- Ensure all client care areas meet Infection Prevention and Control guidelines
- Comprehend and be aware of company policy regarding emergency action plans, fire drills, and disaster awareness.
- Pass quarterly fire and emergency drills conducted by qualified Safety personnel.
