What are the responsibilities and job description for the Home Health RN - Admissions (West Palm Beach) position at AMG Healthcare?
AMG Home Healthcare Services is looking for qualified Registered Nurses interested in a part-time and/or per diem role as a field RN to open Home Health cases (admissions) in the Lake Worth to West Palm Beach area.
Our nurses are responsible for assessing & determining patient's home health need(s),providing establishing the plan of care, and documenting and providing skilled nursing care to our patients; while adhering to confidentiality and professional boundaries.
Responsibilities include:
- Completing OASIS-E Start of Care visit note for each patient admitted.
- Conduct comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and make necessary adjustments to the care plan
- Educate patients and their families on managing their health conditions
- Collaborate with other healthcare professionals to coordinate patient care
- Document all patient interactions and interventions accurately and timely
- Provide skilled nursing care to patients in their homes
Qualifications:
- Valid Registered Nurse (RN) license
- Have previous home health experience
- Active BLS Certification
- Have a reliable method of transportation
- Proficient in conducting home assessments, including vital signs and physical examinations
- Knowledge of anatomy, physiology, and medical terminology
- WellSky/Kinnser Knowledge is a plus
- Bilingual Spanish and/or Creole is a plus
- Ability to perform venipuncture and administer medications intravenously if required
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
