This is a full time position.
Duties
Service delivery – provide ongoing services in accordance with operating procedures, regulatory requirements and best practice standards. (60%)
Facilitates treatment focused skills development individually and in groups in accordance with the ITP.
Oversees and maintains the supervision and safety of children in the Program.
Proactively uses effective de-escalation and behavior management skills. Deals with crisis situations responsibly and effectively.
Provides transportation to and from the Program with Wilder or personal vehicle, as requested.
Present skills training opportunities related to program and participant issues.
Care coordination and referrals – coordinate care or support needs and refer for services as required (5%)
Develop liaison relationships for the child in home, school and community settings in order to promote and enhance the ITP.
Provide current resources and information to program participants and the families. Develop and maintain knowledge of community, education and organizational resources that benefit participants.
Develop professional relationships and maintain a practical understanding of current community resources available to support clients.
Follow up with providers to ensure completion of referrals. Advocate for clients, as needed.
Serve as mandatory reporter to Child and/or Adult Protection regarding issues of neglect and abuse.
Develop and conduct consultation and training for outside agencies and professional, as requested.
Client assessment – provide initial and ongoing screening and assessment as required. (5%)
Assist in the development and implementation of client ITP’s in order to promote emotional and social growth of the child.
Documentation - provide timely, accurate and complete documentation in accordance with operating procedures, regulatory requirements and best practice standards. (20%)
Provides ongoing communication, documentation and reporting regarding child status in order to meet regulatory requirements and Program expectations. BA Counselor – In Home - Page 1 -Monitor and document participant progress, prepare and submit timely progress notes and summaries.
Prepare relevant correspondence and complete forms as necessary.
Engagement – provide participant engagement or program outreach as required. (10%)
Develop professional relationships and partnerships with other community service providers.
Provide community engagement or program outreach as required.
Provide proactive, constructive participation in program meetings.
Job Qualifications
BA degree with five years’ experience as a BA practitioner OR BA with 6,000 hours of clinically-supervised practice OR BA and leadership role in a best-practice or similar service model OR No BA but 10 years relevant experience OR TCM Exemption for services provided to non-English speaking clients.
Minimum of 2 years’ experience in social service, mental/public health field required supervised by an MHP. Experience with family outreach, support and counseling youth groups. If working with child clients must have training working with children. If working adult clients must have training working with adults.
