Duties

The Family Resource Navigator, operating under the general direction of Saint Paul Promise, plays a pivotal role in implementing whole-family support for families in Saint Paul. The Family Resource Navigator will work directly with families from our anchor schools and community partners to provide referrals, resource navigation, and support to meet immediate and short-term goals. All participating families will be offered the opportunity to complete a success plan upon exit from the program to help ensure household stability.

With a strong focus on fostering family engagement, the Family Resource Navigator collaborates with internal staff, Wilder programs, external community resources and partners, schools, and county departments to provide a holistic support system. The Family Resource Navigator will assist and encourage families to achieve their goals and create a stable foundation for their household. By offering support, empowerment, and advocacy, the Family Resource Navigator guides program participants through the complexities of service systems, facilitating their successful navigation. This position is fully funded through a CRC and is contingent upon continued grant funding. While we actively seek sustainable funding sources, this role will conclude when grant funding ends unless alternative funding is secured.

Engagement (40%)- Foster participant engagement and conduct program outreach effectively to support the goals of families and youth enrolled in Saint Paul Promise programming. Responsibilities include but are not limited to:

Develop and maintain trusting and consistent relationships with program participants through calls and in-person meetings (physical or video)

Work with program participants to identify specific resources to help navigate barriers to household stability.

Partner with program participants to create success plans to overcome barriers and obstacles.

Assist families in accessing resources that promote stability, such as requesting assistance from the DHS and identifying local food pantries.

Respect and respond appropriately to each program participant's culture, language, values, and family structures.

Recognizes and supports families' strengths, interests, concerns, and goals.

Establishes trusting relations with program participants to facilitate the accomplishment of individual goals.

Work with other staff at SPPN and community partners in a coordinated team approach.

Develop and nurture positive relationships with community partners, service agencies, providers, and other stakeholders. This entails establishing collaborative connections to enhance the availability and accessibility of resources for program participants.

Maintain a comprehensive understanding of internal and external resources available to program participants.

Contribute to the development and implementation of policies and procedures, ensuring that they are designed using the whole-family approach to meet the specific needs and aspirations of program participants.

Actively conduct outreach and recruitment. Work closely with program staff to identify potential participants, communicate program benefits to families, and support them through the enrollment process if applicable. This may involve conducting outreach activities, providing information sessions, and addressing any questions or concerns.

Service Delivery (30%) – provide services in accordance with the Saint Paul Promise approach/model, operating procedures, regulatory requirements, and best practice standards. Serves as a multiple-service broker for program participants. Assist with the coordination of services by developing multiple partnerships with public/private agencies. Responsibilities include but are not limited to:

Coordinate efforts to support program and program logistics

Identify available community resources for program participants and actively assist in developing a coordination infrastructure for partnerships (internally and externally) that promote whole-family services at both the front line and coordination levels.

Communicate regularly with program participants, among other logistical tasks.

Coordinate and collaborate with school partners and other internal and external partners.

Implement mechanisms to engage program participants and ensure that family voices and perspectives are considered. This may involve conducting surveys, focus groups, or other strategies to gather meaningful input.

Create and maintain a flexible, family-focused, and culturally sensitive resource hub of support for program participants.

Data Management and Project Coordination (30%) – Ensure timeline, comprehensive, and accurate documentation in alignment with operating procedures, regulatory requirements, and best practice standards, incorporating the following:

Maintain meticulous records of support provided and adhere to documentation requirements and timelines specified by the program.

Collect, enter, monitor, track, and maintain all data on program participants served in various systems as defined by the program.

Collect, monitor, and track data on program participant's progress toward their goals in shared database systems.

Adhere to the program's applicable practice standards and protocols, ensuring that all documentation aligns with the organization’s guidelines and quality standards.

Ensure the collection, entry, quality, and security of program participant data.

Assists in coordinating and maintaining communication with families and the community and preparing documents, reports, and written materials.

Other Job Duties:

Serve on committees as requested.

Occasional evening or weekend hours may be required for special events.

Meet with staff individually or in groups, weekly to bi-weekly.

Provide proactive, constructive participation in program meetings.

Develop and maintain professional and technical knowledge and competence.

Make presentations to community groups.

Keep supervisor informed and advised of needs or issues.

Complete staff development opportunities throughout the year in areas of relevance.

Other duties as assigned.

Job Qualifications

Education: Bachelor's degree in human services, behavioral sciences, social work, community health or related field.

Associates Degree in human services, behavioral sciences, community health, social work or related field with three years of relevant experience may substitute for BA degree.

Experience Requirement:

Experience and proficiency with technology and data management systems, including Microsoft Office and email. SalesForce experience is preferred but not required.
Deep commitment to cultural responsiveness and studying oneself.
Experience working with a diverse population.
Superb written and oral communication skills, including meeting facilitation.
Familiarity or experience with Family-centered coaching, motivational interviewing, and behavior change models.
Ability to be detail-oriented, highly organized, and flexible.
Ability to prioritize and complete multiple tasks and projects simultaneously.
Ability to accurately collect, enter, and analyze data.
Excellent time management skills and problem-solving skills.
Knowledge and familiarity with Saint Paul’s Rondo, Frogtown, and Summit-University neighborhoods.
Demonstrated experience with direct client interaction involving low-income households.
Relevant experience and skill sets related to interviewing, conducting assessments, developing and implementing success plans, and care coordination that is inclusive of the entire family are preferred.
