Job Summary:
The Public Health RN conducts home visits for families post-hospital discharge, providing support through maternal and newborn assessments, education on postnatal care, breastfeeding, parent-infant attachment, home safety, and other related topics. The RN helps identify client needs and risks, providing resources to support the family. They will work with pregnant individuals with substance use histories, parents of substance-exposed infants, and families facing various challenges (e.g., domestic violence, homelessness, mental health issues). A trauma-informed approach is used to link families with community resources.
Key Responsibilities:
Conduct home visits to assess postpartum recovery, infant health, and feeding.
Provide education on maternal and child health, breastfeeding, attachment, and home safety.
Assess clients for postpartum complications, depression, home safety, and social support.
Refer clients to medical providers and community resources as needed.
Maintain timely, accurate records, and adhere to confidentiality standards.
Collaborate with other team members and follow program guidelines.
Attend required training, meetings, and supervision sessions.
Provide guidance to the Licensed Vocational Nurse as required.
Minimum Qualifications:
Education: Bachelor's degree in Nursing.
Experience: At least 1 year in maternal and/or newborn services.
Licenses/Certifications: Current CA RN license, Public Health Nurse certification (within 1 year), Certified Lactation Educator (within 6 months), Basic Life Support (BLS) certification.
Language: Bilingual (English/Spanish).
Skills: Intermediate computer skills (Microsoft Office, Zoom), strong communication, and interpersonal abilities.
Preferred Qualifications:
6 months of experience with home visits.
Knowledge of local community resources.
Experience working with diverse, at-risk populations.
Work Requirements:
Full-time position with potential evening/weekend work.
Travel required, valid CA driver's license and insurance necessary.
Background checks, health clearance, and other required clearances.
