Amity Care Hospice is seeking a full-time, experienced hospice RN case manager to care for patients in the Atlanta area. We have been serving hospice patients in the Metro Atlanta area for 14 years. Qualifications:
- Current Georgia RN License.
- Current CPR certification.

Responsibilities
- Conduct comprehensive patient assessments to develop and implement individualized care plans.
- Coordinate with interdisciplinary teams to ensure optimal patient service and discharge planning.
- Maintain accurate documentation in compliance with HIPAA and medical terminology standards.
- Provide education and support to patients and families regarding hospice care options.
- Manage time effectively to prioritize patient needs and case management tasks.
- Utilize strong communication skills to facilitate collaboration among healthcare providers.
Job Type: Full-time
Pay: $80,000.00 - $90,000.00 per year
