34,000 annual patient volume FM or IM Physicians with EM Fellowship or EM experience 24 ED (Emergency Department) beds Scribe coverage EDMIS EMR (Electronic Medical Record) RDU / Observation Unit run by the ED SUBMISSION REQUIREMENTS
Must have NY license and DEA (Drug Enforcement Administration) at the time of presentation Must have clean malpractice / background for temporary privileges Must be Board Certified - Prior to Name Clear Must send self-query NPDB and EPA (Envision Pre-Application) with presentation Must submit vaccination card with presentation Must have 2 years ED (Emergency Department) experience AS A REMINDER :
All candidates must have met the minimum shift availability listed on the job. CV (Curriculum Vitae) must have all dates in MM / YY to MM / YY format Work History must be in Sequential order with most resent-on top Any Gaps over 30 days must be explained on the CV EPA (Please use the attached EPA. It is also in Trio under your Name & then click on Help) All Licenses, Boards and Certs with expiration dates must be listed on CV Anyone with malpractice must provide a NPDB Anyone with legal issues must provide supporting documents Availability
1st date they would be available? How many shifts would they be able to do per month? What month is their schedule completely open? Any shift preferences (Day, Night, Blocksetc.)?
