POSITION SUMMARY – RN Case Manager – Home Hospice
POSITION DUTIES – Provides skilled nursing, care, and case management for assigned patients in their place of residence; Admits patients onto services; Coordinates and integrates services to ensure continuity of care; Facilitates the achievement of quality, clinical and cost outcomes; Negotiates, procures and coordinates services and resources needed by the patient or family; Creates opportunities and systems to enhance outcomes; and uses the nursing process of assessment, planning and implementation of care, including patient/family instruction and evaluation through an inter-disciplinary team approach; May start as a Visiting RN and move to Case Manager.
MINIMUM REQUIRED QUALIFICATIONS – MT/compact RN license; BLS; Valid driver’s license with appropriate auto insurance & reliable transportation.
PREFERRED QUALIFICATIONS – BSN; 2 years of recent case manager, acute care, home care, or hospice experience including such clinical skills as venipuncture, catheterization, wound care and/or parenteral therapy.
LENGTH OF ASSIGNMENT – 12 weeks
SHIFT / HOURS PER WEEK – M-F, 8-5 with Call (40hrs/wk)
