POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – WA/compact RN license; BLS; 2 years as discharge planner, 4 years Acute Care RN experience
PREFERRED QUALIFICATIONS – BSN; Healthcare financial and reimbursement knowledge preferred, Leadership, ED, and Utilization Review experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8a-4p, every other weekend
SYSTEMS – Epic
