POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; BLS; 1 year acute hospital case management experience; Acute hospital experience for the past year with no more than a 90-day break; Meditech/Midas experience
PREFERRED QUALIFICATIONS – Complex discharge planning experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8:30A-5P. Every other weekend and holidays in rotation.
SYSTEMS – Meditech/Midas
