POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – CA RN license; BLS; 1-2 years acute care hospital Case Manager experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – Primarily M-F with occasional weekend coverage
SYSTEMS – UDS ProDoc and ProTouch
START DATE – ASAP
