POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – MO/compact RN license; BLS; 2 years acute hospital case management experience; Recent Acute Care Discharge planning experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – M-F 8-4:30 rotating weekends
SYSTEMS – Meditech, Midas, Wellsky
START DATE – ASAP
