POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – AZ/compact RN license; 2 years case management experience in an inpatient setting
PREFERRED QUALIFICATIONS – Utilization review & discharge planning experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – M-F 8am-5pm (Weekly Guaranteed Hours: 36)
SYSTEMS – Cerner, UDS Pro Doc, Meditech
START DATE – ASAP
