POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – GA/compact RN license; BLS; 1 year acute care Case Management experience; Care Coordination experience; Interqual proficiency.
LENGTH OF ASSIGNMENT – 12 weeks
SHIFT / HOURS PER WEEK – M-F 8a-4:30p with rotating weekends (Sat & Sun 8a-6p)
SYSTEMS – MIDAS, EPIC, Careport
START DATE – 3/3/2025
