POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Floating among sister campuses required
MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; BLS; Accredited nursing degree; 2 years acute hospital case management experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8hr Days (40hrs/wk)
