POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; 1 year acute care hospital case management experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8:00AM - 4:30PM / 40hrs/wk
SYSTEMS – Meditech, Midas, Wellsky
START DATE – ASAP
