POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; BLS; 1 year acute hospital case management experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – M-F, weekends as needed
SYSTEMS – ProDoc, ProTouch
START DATE – ASAP
