Position Overview:
We are currently seeking a compassionate, detail-oriented, and experienced Registered Nurse (RN) to join our home healthcare team as a Supervisor. In this pivotal role, the successful candidate will oversee caregivers and ensure the delivery of high-quality care to patients in their homes. The RN Supervisor will work closely with patients, families, and a multidisciplinary team to coordinate and improve home healthcare services.
Key Responsibilities:
**Patient Care Supervision:** Oversee the assessment, planning, implementation, and evaluation of patient care plans in collaboration with a healthcare team. Ensure services meet patient-specific needs.
**Caregiver Oversight:** Supervise and evaluate the performance of caregiving staff. Provide guidance, training, and support to caregivers to enhance their skills and ensure high standards of care.
**Quality Assurance:** Implement and monitor compliance with healthcare policies, procedures, and standards. Conduct regular audits to ensure adherence to regulations and quality benchmarks.
**Care Coordination:** Facilitate communication among patients, families, caregivers, and healthcare professionals to ensure cohesive care plans. Adjust plans as necessary based on patient progress and feedback.
**Emergency Response:** Be readily available to respond to urgent issues or changes in patients' conditions. Provide direct care or instructions to caregivers in emergency situations.
**Documentation and Reporting:** Maintain accurate and up-to-date patient records. Compile reports on patient outcomes, caregiver performance, and program effectiveness for review by healthcare management.
**Professional Development:** Stay informed of advancements in home healthcare practices. Participate in professional development opportunities to improve knowledge and skills.
Qualifications:
- Current RN license in the state of practice.
- Demonstrated ability to lead and mentor a team.
- Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare professionals.
- Strong organizational and time-management skills.
- CPR certification and additional certification in home healthcare nursing preferred.
Working Conditions:
