Care Coordinator/Community Health Worker (CHW)

Reports to: Nurse Care Manager, Health Services

Overview
The Care Coordinator/CHW will act as a bridge between the Amos House Outreach team and Amos House Clinic, supporting individuals who come into Amos House for emergency services and connecting them with behavioral health and medical services, as needed. The Care Coordinator will meet with people who walk in for services at Amos House to assess their needs, directly address needs related to benefits and housing, and facilitate connections to Amos House and other health care providers, including for evaluation for social security benefits. The Care Coordinator will also support Clinic clients in consistently engaging in treatment, including assisting with transportation, locating people who have been lost to follow-up, and helping clients navigate pharmacy requests.

Duties

• Conduct assessments to identify health-related social needs, as well as basic health and behavioral health-related screenings.
• Assist with benefits enrollment and with obtaining IDs, birth certificates, and social security cards as needed.
• Preparing clients to enroll CES with all required assessments and documents and completing housing applications
• Assist clients in making medical and behavioral health appointments, including providing appointment reminders and transportation. ac
• Attending appointments with clients as requested.
• Supporting people in applying for SSI/SSDI, including coordinating with medical providers and lawyers.
• Conducting outreach to locate clients who have fallen out of care and/or to communicate test results and other medical follow-up.
• Coordinating care with other providers and case managers
• Be able to provide positive modeling of behavior, language, promptness, and responsibility for caseload.
• Sharing lived experience with clients in a professional manner (as relevant)
• Document case notes in AWARDs and complete all necessary documentation for Medicaid billing.

Requirements

• Certified in RI as a CHW (community health worker) or have completed the number of training hours required for certification.
• Proficient in Microsoft office and able to enter written notes in our electronic medical record.
• Must be able to maintain confidentiality in all aspects of the work environment
• Must have an understanding and respect for everyone’s unique path to recovery; and be sensitive to adults with diverse educational, socio-economic, and cultural backgrounds, including individuals who are homeless, in recovery, or formerly incarcerated.
• Must have a working knowledge of health disparities that include behavioral health and demonstrated commitment to the recovery community.
• Bilingual Spanish-English preferred.
