What are the responsibilities and job description for the Reentry Support Specialist position at Amos House?
About the Role
At Amos House, we believe in the power of second chances and the strength of community. As a Reentry Peer Support Specialist, you’ll provide essential support to clients transitioning from incarceration back into the community, helping them build a foundation for stability, recovery, and success.
In this role, you’ll work one-on-one with clients returning from the Adult Correctional Institutions (ACI) to Providence, many of whom are navigating substance use recovery and other challenges. You’ll help them develop personalized wellness and reentry plans that focus on securing housing, employment, healthcare, and family support—ultimately reducing the risk of relapse and reincarceration. Your support will include coordinating services, making referrals, and providing hands-on assistance both before and after their release.
In this role, you will embody the mission, vision, and values of Amos House, helping people to help themselves by engaging returning clients in a holistic, strengths-based, trauma-responsive, and person-centered manner.
Duties/Responsibilities:
- Provide one-on-one peer support and care coordination for a caseload of clients.
- Facilitate warm handoffs with reentry partners and service providers.
- Ensure clients obtain vital documents such as birth certificates, Social Security cards, state IDs, and public benefits (SNAP, health insurance, SSI/SSDI if eligible).
- Build strong, trusting relationships with clients and their support networks.
- Train clients in NARCAN administration and ensure they and their caregivers have access to it.
- Assess client needs, develop individualized reentry case plans, and document progress toward goals in housing, employment, recovery, and family reunification.
- Complete the Multi-Dimensional Inventory of Recovery Capital as needed.
- Maintain accurate case notes and documentation in AWARDS (our electronic medical records system).
- Provide wraparound services both on-site and in the community.
- Offer transportation to appointments and essential services, as necessary.
- Attend NA/AA and other recovery-focused meetings alongside clients for additional support.
- Perform other duties as assigned.
