Our team of healthcare professionals positively impacts the health, well-being and quality of life for those we serve providing comprehensive, integrated, affordable healthcare and education to patients. We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.
The Adolescent Health Educator is responsible for supporting the development, implementation, and evaluation of the Personal Responsibility Education Program (PREP), facilitating an ongoing Get Real curriculum targeting at-risk adolescents in Manchester.
Job Relationships:
- The Adolescent Health Educator reports to the Adolescent Health Education Supervisor
- Other relationships: Works closely with other PREP Educators Advancement staff, Medical Providers, Behavioral Health Clinicians, and perinatal clinical staff.
- High School Diploma or equivalent , Spanish or other language skills preferred.
- Some Experience in public speaking or working with adolescents preferred
- Some experience in sexual health education preferred but not required
- Social media experience preferred
- Occasional weekend ability required
Compensation: The starting rate for this position is $19.00 per hour, which reflects the beginning of the pay scale. At the time of an offer, determination of your rate will reflect your skills and experience as it relates to the position.
Responsibilities:
- Facilitate ongoing PREP classes using an evidence-informed curriculum designed to provide skill building to at-risk adolescents in Manchester.
- Support the implementation of an evidence-informed pregnancy and STI prevention educational program.
- Support the Adolescent Health Education Supervisor in the development and implementation of community outreach programs to promote the program and general pregnancy and STI prevention education with both service providers and potential clients, especially targeting hard-to-reach populations.
- Support development and publication of educational outreach via social media.
- Collect PREP enrollment data for state and federal reporting purposes.
- Assist the Supervisor in creating quarterly grant reports in accordance with PREP funding requirements.
- Maintain privacy and confidentiality in relationships with adolescents and record keeping. Display professional behavior when working with adolescents, parents, and community agencies.
- Attend community and special events as requested by the Supervisor. These may require attendance outside of normal business hours.
- Other duties as assigned.
