We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.
The Behavioral Health Case Manager assists the Behavioral Health team in providing coordinated behavioral health care to patients in an integrated primary care setting. Functions include: assessing clients' needs, facilitating access to various resources, and advocating for clients to ensure they receive appropriate care and support. The case manager works collaboratively with healthcare providers, community resources, and patients to promote overall well-being and recovery.
