We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Manager, Care Coordinators, and Community Health Workers.
Psychiatric Nurse Practitioners (Psych NP’s) at Amokeag Health provide psychosocial evaluation, diagnosis, and therapeutic interventions for patients with various behavioral health issues. This may include medication management, brief interventions, assessments and referrals. The Psych NP’s at Amoskeag Health staff what is primarily a consultative service. The primary care providers (PCP’s) refer patients to the Psych NP’s by way of the counseling staff and the main reasons for referrals are either clarification of a psychiatric diagnosis or diagnoses or assistance with a medication plan of care for the patient. The Psych NP’s see the patient for 3-5 visits on average and then return the patient to their PCP for routine f/u care. Pediatric Psychiatric Nurse Practitioners see patients ages 8 and older.
Compensation:
The starting rate for this position is $61.50 per hour, which reflects the beginning of the pay scale. At the time of an offer, determination of your rate will reflect your skills and experience as it relates to the position.
Responsibilities:
- Psychiatric Nurse Practitioners provide leadership and shape the clinical direction of our program by providing bio-psychosocial evaluation, diagnosis and therapeutic interventions while working on a multi-dimensional team. Duties my also include evaluation treatment services including, medications management, counseling, brief treatment interventions, substance abuse assessments, treatments and referrals. A Master of Science in nursing and a certified psychiatric ARNP license is required with the ability to prescribe medication in New Hampshire.
- Make appropriate referrals to both internal disciplines of Amoskeag Health and/or external agencies for additional services required, including follow-up assessment of a medical condition by primary care physician, sub-specialty evaluation, social services, substance abuse, and mental health counseling.
- Provide care to clients with acute psychiatric symptoms, therefore, Nurse Practitioners’ schedule will allow for clients to be seen on a same day basis as necessary. Nursing staff will determine which clients are in need of same day appointments in collaboration with the Psychiatric Nurse Practitioner.
- Provide appropriate follow-up with assistance from the nursing staff, as needed, to include telephone/written correspondence (i.e., to inform clients of abnormal test results). Consult with nursing staff as indicated.
- Assist with the development of clinical policies/protocols for the patients as relates to behavioral health services.
- Provide individual and group health education to clients and families.
- Provide consultation to nurses who triage client telephone calls and walk-ins. Refer clients for specialty services.
- Maintain accurate, timely documentation in clients' electronic medical records of clients' contacts, case planning, and plan of care. All documentation in a client's electronic medical record to be of a nature that can be understood by anyone who may need to refer to that client record.
- Provide clear written and oral communication to both Amoskeag Health clients and employees.
- Provide outreach/consultation to other community agencies/groups when appropriate
- Participate in case conferencing/management to assist in the development of the clients' health care plans.
- Attend and participate in various clinical & administrative meetings, as determined by the Medical Director, and other Amoskeag Health meetings as directed. If absent, responsible for reading minutes of meetings and signing the documentation.
- Participate in meeting requirements of Amoskeag Health funding sources as needed. This may include, but is not limited to assisting with development, implementation, and monitoring of grant proposals.
- Attend mandatory safety-related and infection control in-services as designated by the Senior Management Team.
- Develop and maintain clinical/Nurse Practitioner policies & procedures with collaboration from other Amoskeag Health staff.
- Participate in other administrative/clinical meetings and short/long term projects that impact the quality/effectiveness of Amoskeag Health services as requested.
- Participate in the data collection and the future planning, for Amoskeag Health from his/her area of expertise.
- Maintain professional development, utilizing state and private consultants. Maintains licensure for Nurse Practitioner status by ongoing professional education and re-certification exams as required by the State of New Hampshire.
- Prescribe medications for clients for treatment of medical conditions, when indicated.
- Sensitivity to cultural diversity of population being served. Maintain client age-related competence.
- Other responsibilities as assigned by supervisor.
Qualifications:
- Education/Training: A MS in nursing and a certified psychiatric ARNP license is required with the ability to prescribe medication in New Hampshire.
- Experience: One to five years' experience as a Psychiatric Nurse Practitioner in a multi-disciplinary team providing primary health care to medically indigent families preferred. Preference is given to candidates who have worked with pediatric patients (ages 8 and up) as well as adolescent and adult patients.
