Our team of healthcare professionals positively impacts the health, well-being and quality of life for those we serve providing comprehensive, integrated, affordable healthcare and education to patients. We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.
The Team Nurse assumes responsibility for nurse visits at Amoskeag Health. They triage all telephone calls and walk-in patients with medical questions / concerns. The Team Nurse position also coordinates the flow of patient communications regarding such things as labs, refills, dosages, changes, and instructions from providers.
Requirements
QUALIFICATIONS :
- Education / Training : RN with 3 years experience. Current NH RN license.
- Experience : Community health experience preferred
- BLS Certification Required
- Knowledge / Skills : Ability to delegate tasks to other team members. Health assessment skills and coordination and delegation skills necessary.
