Job Summary: 

The Home Visitor (HV) designs and implements a collaborative and supportive program, based on the Company defined curriculum, with clients that focuses on developing parenting skills in support of healthy child growth and development. HV builds positive and supportive relationships with clients and provides education and resources for a variety of topics and addresses individual needs. HV performs regular assessments to evaluate client needs and determine if developmental milestones are being achieved. HV acts as an advocate for families and will carry out all duties, tasks, and assignments in a confidential, competent, and professional manner within established deadlines, in compliance with all legal, the New Mexico Home Visiting Program standards, and Company requirements, under the direct supervision of the HVPD.


Qualifications:

  • HS Diploma, GED or equivalent degree and a minimum of two years of experience working with infants, toddlers, preschoolers, or their families, with a commitment to obtain the Infant Family Services Certificate or Community Health Worker Certificate within 3 years of hire; OR
  • An Associate’s degree in Early Childhood Education or a closely related field, with a commitment to obtain the Infant Family Services Certificate or Community Health Worker Certificate within 2 years of hire; OR
  • A Bachelor’s degree in Early Childhood Education or a closely related field.
  • Ability to pass a background check through the New Mexico Early Childhood Education & Care Department (ECECD).
  • Must have reliable transportation to complete bi-monthly home visits throughout assigned county.
  • Proficiency in Microsoft Windows and Microsoft Office platforms.


Training and Supervision Requirements:

  • Completes all necessary ECECD and Parents as Teachers (PAT) trainings including participation in 12 hours of Professional Development and monthly reflective supervision requirements of the New Mexico Home Visiting Program as part of an Individualized Professional Development Plan. 
  • Completes all Amplified Therapy, Inc. (ATI) mandatory training as assigned.

 

Duties and Responsibilities:  

  • Committed to fulfilling Company's Mission, Vision, and Values.
  • Demonstrates good communication skills.
  • Conducts home visits with pregnant women and/or parents with children ages 0-5 years developing trusting and confidential relationships with each parent and family in accordance with defined requirements.
  • Presents educational material on a variety of topics related to pregnancy, child development, care of infants and toddlers, mental and physical health, nutrition, and household management.
  • Assists families in accessing resources, learning how to support child growth and development, and implementing strategies on material presented.
  • Evaluates functioning/developmental level of child and parent to tailor educational program to specific needs and to make appropriate referrals for formal assessment.
  • Teaches and assists parents to evaluate and assess their child’s developmental level.
  • Supports socialization experiences for families providing resources and information about events, parent workshops and meetings.
  • Implements the appropriate performance standards for ECECD home visiting.
  • Does regular assessments to identify needs for community or clinical resources for mother, infant, and other family members.
  • Provides crisis intervention when needed.
  • Acts as support person and advocate for families.
  • Enters data accurately and in a timely manner, protecting the integrity and accuracy of information.
  • Documents and evaluates any concerns regarding substance abuse, or possible child neglect or abuse, and makes appropriate referrals.
  • Maintains a current and confidential file for each family with all-pertinent information, documentation, referrals, etc.
  • Provides quality service within the Home Visiting Program that meets Company’s standards, including quality documentation and professionalism.
  • Participates in Company Marketing Plan and assists with organization and execution of community outreach efforts to generate targeted referrals.    
  • Complies with New Mexico Home Visiting Program requirements.
  • Attends required New Mexico Home Visiting meetings/trainings.
  • Maintains proficiency in Home Visiting services by attending workshops, conferences, and trainings.
  • Responds to emergency situations in a competent manner.
  • Adheres to established Company guidelines within the Company’s Official Documents.
  • Maintains assigned equipment in proper working order.  
  • Performs other duties as assigned.

 

Work Conditions/Physical Requirements:

  • Due to the nature of the Company, service delivery settings will vary. 
  • Work area may be crowded.
  • Working schedule may vary due to the nature of the business.
  • Duties may involve being on one’s feet for extended periods.
  • Duties may involve traveling to and from areas within the western region of New Mexico.
  • Duties may involve exposure to hazardous conditions using power tools, contagious diseases, and chemicals to repair/adjust patient equipment.
  • Must be able to lift a minimum of 50 lbs.
