Job Summary: SW is responsible for the service delivery process in development, implementation, and coordination of social work services. Social Work Services are provided as designated in the Individualized Family Service Plan (IFSP) that include identifying, mobilizing, and coordinating community resources and services to enable the family/child to receive maximum benefit of early intervention services; helping the parent(s) understand the special needs of their child; preparing a social or emotional developmental assessment of the child within the family context; making home visits to evaluate patterns of parent-child interaction and the child’s living conditions, providing individual and family-group counseling with parents and other family members, and appropriate social skill-building activities with the child and parents; and working with those problems in a family/child’s living situation that affect the child’s maximum utilization of early intervention services.
- Minimum Education requirements:
- Bachelor’s degree in Social Work from a CSWE accredited program, if licensed as a LBSW.
- Master's degree in Social Work from a CSWE accredited program, if licensed as a LMSW or LISW
- Licensure from the New Mexico Board of Social Work Examiners

- Provides targeted, evidence-based therapeutic intervention to facilitate patient participation and success in therapy.
- Participates in multidisciplinary meetings to review evaluation results, integrate findings with other disciplines, offer recommendations, and develop individual treatment programs.
- Monitors and reassess the effects of social work services and the need to continue, modify, or discontinue intervention.
- Teaches, monitors, and collaborates with support personnel and family/child to facilitate skills in areas of development as well as health maintenance and safety.
- May represent the Company while participating in community outreach events, activities, meetings, trainings, etc.
- Participates in team discussions in an articulate, positive, and productive manner.
- Maintains assigned equipment in proper working order. Perform routine repairs.
- Instructs staff, as assigned, in principles used in social work services.
- Completes all documentation in a timely manner.
- Attends departmental/business staff development activities.
- Maintains proficiency in social work by attending workshops, conferences and trainings.
- Attends all mandatory in-services and participate in Company committees as requested by supervisor.
- Responds to emergency situations in a competent manner.
- Adheres to SW licensure requirements.
- Adheres to established Company guidelines within Company’s Official Documents.
- Assists in quality management, research, public relations, and program development projects when appropriate.
- Participates in Company’s Professional Development Plan.
- May be assigned to be a clinical supervisor for assistant level students.
- Carries out all responsibilities professionally and efficiently.
- Maintains effective communications at all levels; within the work setting and company.
- Perform other duties as assigned.
- Due to the nature of the Company, service delivery settings will vary. Must be able to adapt to a variety of settings and be willing to work in clinical, community, and home settings.
- Work area may be crowded.
- Working schedule may vary due to the nature of the business.
- Duties may involve being on one’s feet for extended periods.
- Duties may involve traveling to and from areas within the western region of New Mexico.
- Duties may involve exposure to hazardous conditions using power tools, contagious diseases, and chemicals to repair/adjust patient equipment.
- Must be able to lift a minimum of 50 lbs.
