Overview:
We are seeking an experienced and dedicated Manager of Case Management to lead our hospital acute care team in Fort Pierce, FL. The ideal candidate will possess either a valid Registered Nurse (RN) license or a Masters in Social Work (MSW), with a strong background in acute care settings. This leadership role involves coordinating patient care, optimizing resource utilization, and ensuring high-quality outcomes for our patients.

Position Type:Full-Time (Permanent)
Pay Range: $80,000.00 - $120,000.00 per year
Key Responsibilities:
- Supervise and mentor the case management team, fostering a collaborative and supportive environment.
- Oversee daily operations, ensuring compliance with hospital policies and regulatory standards.
Patient Care Coordination:
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Facilitate patient transitions across the continuum of care, minimizing delays and optimizing resources.
Quality Improvement:
- Monitor case management metrics and outcomes, implementing strategies for continuous quality improvement.
- Participate in hospital-wide initiatives to enhance patient care and operational efficiency.
Education & Training:
- Provide ongoing education and training to staff on case management best practices and documentation standards.
- Serve as a resource for complex case consultations and patient/family education.
Compliance & Documentation:
- Ensure accurate and timely documentation of case management activities in compliance with hospital and regulatory guidelines.
- Conduct audits and reviews to maintain high standards of practice.
Qualifications:
- RN with current Florida license or Masters in Social Work (MSW) from an accredited program.
Experience:
- Minimum of 3-5 years of case management experience in an acute care setting.
- Prior leadership or supervisory experience preferred.
Certifications:
- Case Management certification (CCM or ACM) preferred but not required.
