While experience with the PACE (Program of All-Inclusive Care for the Elderly) program is preferred, we welcome candidates with a strong background in geriatrics and a willingness to learn.
Responsibilities:
- Conduct thorough medical examinations and assessments of geriatric patients to establish accurate diagnoses.
- Develop and implement personalized care plans, considering the unique needs and conditions of elderly individuals.
- Collaborate with interdisciplinary teams, including nurses, social workers, and other healthcare professionals, to ensure holistic and coordinated patient care.
- Provide ongoing medical supervision, monitoring, and adjustments to treatment plans as necessary.
- Educate patients and their families on preventive care, treatment options, and lifestyle modifications to enhance overall well-being.
- Maintain accurate and up-to-date electronic medical records, documenting patient history, progress, and treatment plans.
- Participate in regular case conferences and team meetings to discuss patient care strategies and coordinate services effectively.

Qualifications:
- Licensed NP
- Proven experience working with geriatric patients, with a focus on providing patient-centered care (preferred).
- Familiarity with the PACE program is preferred but not required; willingness to undergo training if necessary.
- Excellent communication and interpersonal skills to engage effectively with elderly patients and their families.
- Ability to collaborate in a team-oriented environment with a patient-centered approach.
