Job Type: Full-Time
Shifts Available:
- Day Shift: 8:00 AM – 4:30 PM (Rotating Weekends)
- Evening Shift: 12:00 PM – 8:30 PM (Alternative shift: 11:00 AM – 7:30 PM, Monday – Friday, No Weekends)
Job Summary:
We are seeking a dedicated and dynamic Case Manager RN to join our team. The RN Case Manager plays a crucial role in coordinating patient care, ensuring appropriate medical necessity, managing hospital admission status, and facilitating efficient discharge planning. This position is instrumental in promoting quality patient outcomes, improving patient throughput, and minimizing barriers to care.
Key Responsibilities:
- Conduct initial and ongoing assessments of patient conditions, level of care, and overall plan of care.
- Review admission and preoperative orders to ensure all necessary components are in place.
- Assist physician offices in obtaining required assessments before procedures, surgeries, or admissions.
- Collaborate with patients and families to provide education and facilitate care transitions.
- Develop and implement critical pathways to optimize patient care.
- Utilize computerized decision-support tools to monitor patient status and potential outliers in length of stay.
- Communicate benefit plans, coverage changes, and financial implications to physicians, patients, and families.
- Manage payor denials and issue hospital-initiated notices of non-coverage (HINN’s).
- Identify clinical outcomes, set target dates, and create a plan for necessary services and care across the continuum.
- Conduct utilization reviews, evaluate clinical information, and provide timely updates to physicians and insurance providers.
- Submit clinical documentation to insurance companies to ensure authorization of hospital stay.
- Document case management activities accurately in electronic medical records.
- Participate in interdisciplinary team rounds to coordinate patient care.
- Assist in discharge planning by ensuring proper transition to the next level of care.
- Address patient and family education needs, considering language and cultural barriers, and integrate them into the care plan.
Qualifications & Requirements:
- Experience:
- Recent acute care case management experience within the last year preferred.
- Will consider candidates with 2-3 years of ICU or strong Med-Surg experience.
- Case management experience in a long-term acute care (LTAC) facility is also considered.
- Education:
- Graduate of an accredited school of nursing (RN); BSN preferred.
- Licensure & Certifications:
- Active RN license (Florida or approved multistate license required; compact license holders must obtain a Florida license within 60 days of relocation).
- Basic Life Support (BLS) certification (AHA Provider issued card required within 30 days of start).
- Certification in Case Management, Nursing, or Utilization Review preferred.
- Skills:
- Strong critical thinking and problem-solving abilities.
- Excellent communication and multitasking skills.
- Ability to adapt quickly in a fast-paced environment.
Compensation & Benefits:
- Salary Range: $32.91/hr - $45.25/hr (Based on experience, with max pay for 10 years of experience).
- Shift Differentials: Evenings - $2.00/hr, Weekends - $2.00/hr.
- Sign-On Bonus: $15,000.
