Job Summary
The Social Services Designee/Activities Director is responsible for planning, implementing, and overseeing engaging programs and activities that promote the well-being and development of individuals in our care. This role requires a compassionate leader who can foster a supportive environment while ensuring compliance with relevant regulations and standards. The Activities Director will work closely with staff, families, and community partners to enhance the quality of life for participants through meaningful engagement. As well, the Social Services Designee will serve as a liaison to residents and their families, making marketing outreach contacts with local discharge planners, home health agencies, etc..
Duties
- Develop and implement a comprehensive activities program that meets the needs of participants.
- Coordinate crisis management strategies to ensure the safety and well-being of all individuals involved.
- Collaborate with community resources, including Medicaid and Medicare services.
- Monitor and evaluate the effectiveness of activities, making adjustments as necessary to improve outcomes.
- Maintain accurate records of participant involvement in activities and report on progress as required.
- Train and supervise staff in best practices.
- Participate in the referral process for new admissions
- Completes admission paperwork and assessments on incoming residents.
- Completes ongoing assessments through the MDS process.
Experience
- A background in social work or related fields is preferred, with experience in long term care or behavioral health settings being highly desirable.
- Demonstrated skills in crisis management and mentoring are essential for success in this role.
- Previous experience working with diverse populations, including elder adults and families, is a plus.
- Strong communication skills are necessary to effectively collaborate with team members, families, and community partners.
