We are looking for a dedicated, accountable, and self-motivated Care Manager to join our team and support clients on their journey to holistic well-being. In this role, you will collaborate closely with clients and their care teams, providing guidance, advocacy, and essential resources to ensure they receive comprehensive, personalized care through our Whole Person Care program.
Key Responsibilities:
- Work with clients in both group and individual settings to identify barriers to care and develop personalized plans for sustainable health improvements.
- Advocate for clients in complex situations, facilitating resolutions while upholding ethical standards and promoting the System of Care principles.
- Assess client needs and provide education on health literacy, self-care skills, and informed decision-making.
- Assist clients in accessing resources and building skills necessary for long-term self-management and wellness.
- Collaborate with the care team to support clients in setting and achieving self-management goals.
- Maintain accurate documentation of client interactions and care coordination efforts.
- Stay updated on local resources and facilitate connections between clients and community services.
- Conduct outreach to enroll eligible members into the Whole Person Care program.
Requirements:
- High School diploma or equivalent; background in social services preferred.
- Experience in counseling or coaching operations preferred.
- Knowledge of health and wellness topics such as nutrition, stress management, and mental health.
- Certification from an accredited professional coach training program is a plus.
- Strong verbal and written communication skills.
- Valid California Driver’s License, clean driving record, and reliable transportation.
- Proficiency in computer applications including MS Office, Electronic Health Records, and databases.
- Willingness to undergo background checks, physical examinations, and TB testing.
