Job summary: Anchor Health is seeking a Medical Case Manager to conduct psychosocial assessments, support patients in accessing social services, and coordinate care for medical, mental health, and substance abuse needs. Responsibilities include managing case documentation, researching resources, and advocating for clients. A Master of Social Work (MSW) or 3-5 years of case management experience is preferred, along with knowledge of HIV/AIDS resources and the LGBTQ community. Bilingual skills (English/Spanish) are a plus. The position is split between our two offices in Hamden, CT and Stamford, CT and requires on-site presence Monday through Friday.
Essential functions:
- Conduct psychosocial assessments including history of trauma, substance abuse, cognitive functioning, and activities of daily living
- Provide information and support to Anchor Health patients for housing, social services, benefits, transportation, public assistance, food/groceries, and other necessary social services
- Advocate for patients in legal, education, and other healthcare settings
- Independently identify patient needs, strengths, and readiness to change
- Streamline care coordination to improve patients' access to services including medical, mental health, and substance abuse programs
- Conduct ongoing assessment of client eligibility for public health and assistance programs and assist in enrollment
- Maintain and update necessary data in the medical case management and HIV case management spreadsheet of client requests and tasks
- Coordinate necessary documents for public and private assistance programs, such as CADAP, CIPA, Advancing Access, and others
- Research resources for case management clients that are LGBTQ-affirming
- Maintain complete and accurate files documenting all care and services provided in the client’s MyChart and all other required documentation methods
- Participate in staff meetings, agency meetings, planning meetings, and other meetings as needed
- Comply with requirements of OSHA, HIPAA, and other applicable regulations
Requirements:
- Master’s degree in social work (preferred) OR 3-5 years of case management experience
- Experience working with the LGBTQ community and familiarity with issues relevant to LGBTQ people and health
- Knowledge of CT social service and benefit systems: Husky, 211, DSS, SNAP, UniteCT, SSDI, etc
- Demonstrates knowledge of effects of psychosocial needs, trauma history, and cognitive/behavioral/motivational functioning on health-related behavior and exhibits ability to intervene appropriately and effectively
- Comprehensive knowledge of HIV/AIDS community resources, and public and private HIV/AIDS benefits programs
- Excellent verbal and written communication skills, including strong organizational, detail, and interpersonal skills
- Excellent Computer skills and knowledge of Microsoft Office, word processing, Electronic Health Records, and other software systems
- Ability to be self-motivated and dedicated to initiating and completing complex tasks
- Ability to handle multiple tasks, be detail-oriented, and maintain strict confidentiality of medical information
- Able to work well independently with minimum supervision
- Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment
Preferred requirements:
- Master's degree in Social Work
- Spanish language fluency
- 2 years of experience providing case management services to the LGBTQ community
- 2 years of experience providing case management services to people living with HIV
