What are the responsibilities and job description for the Hospice RN Case Manager position at Anchor of Hope Hospice?
Hospice RN case manager will oversee all aspects of patient care. The case manager will provide assessments, manage medication needs, medical equipment needs, coordinate individualized care plans, and support the patient and family. The RN case managers provide nursing services for new admission and ongoing patient hospice care. In addition to providing nursing services the case manager communicates to the medical director and patient/ patient representative changes in condition and changes to service plans.

Pay: $88,000.00 - $90,000.00 per year
