GENERAL: The Texas Correctional Office on Offenders with Medical or Mental Impairments (TCOOMMI) Adult Probation Intensive Case Manager (ICM) will be responsible for providing person-centered intensive case management services to adults diagnosed with severe and persistent mental health disorders. This position must possess a passion and aptitude for working with individuals who have chronic and severe mental health needs and are involved in the criminal justice system. Knowledge of psychotherapeutic medications, intensive case management functions, substance use treatment, crisis services, community resources, and evidence-based practices in the mental health and criminal justice systems are required. This position must use innovative thinking, exercise patience and be resilient. Skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning and dual-diagnosis treatment are preferred. This position will effectively work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training opportunities, balance self-care, and perform other duties as assigned with a positive attitude. Other Job Functions: Promote and maintain a Trauma Informed, Person Centered, environment for all clientele and staff. Ensure each individual receives no less than 3.5 monthly hours of behavioral healthcare services accordance with the TDCJ TCOOMMI PGP 01.02. Regularly communicate to Program Director with feedback, concerns and solutions to meeting expectation. Initiate contact with individuals served and/or the Legal Authorized Representative to assess needs, provide collaborative and person-centered recovery planning, facilitate referrals and linkages to appropriate resources, monitor service effectiveness, provide advocacy, facilitate crisis intervention or prevention services, as needed and within a timely manner and in accordance with HHSC and TCOOMMI contract/guidelines. 2 Initiate and reciprocate coordination and collaboration with members of the Recovery Team and/or other Intensive Recovery Teams, Program Director, Center Directors, peer specialists and other ACCESS employees/departments, and community members, as appropriate, to better provide quality services and support service delivery. Coordinate intake and assessment of individuals referred to the TCOOMMI program. Ensure that clinical documents and tools necessary for state contracts are completed electronically and timely to assure services are clinically appropriate and documented. This includes completion of, but not limited to, uniform assessment (ANSA), PersonCentered Recovery Plans, Demographics, Diagnosis, Financial Assessments. Ensure all individuals have a current Person-Centered Recovery Plan (PCRP) that identifies intensive case management as an intervention. Identify the psychosocial needs of the individual and facilitate their attendance to related services Identify and coordinate the individual’s access to all available and therapeutic and behavioral health services including inter-and intra-agency resources and serve as an advocate for the individual. Provide individual service in the areas of activities of daily living, coping skills, problemsolving, stress and anger management, social and recreational skills, psychiatric symptom management, effective communication and psycho/social functioning recovery Ensure all service activities are clearly documented in the electronic health record and documentation indicates interventions performed, progress made, and relation of services to recovery plan goals. Provide purposeful, thoughtful transitional and/or discharge services with approval and consultation of the Program Director and/or with otherwise assigned clinical staff, as appropriate. Develop and maintain strong, professional, and effective collaborative working relationships with probation and other community agencies Develop and maintain strong, professional, and effective collaborative working relationships with treatment team members. Provide collaborative and concurrent documentation at least 95% of the time. Document all services to include kept appointments, no-shows, cancellations by individuals and self. 3 Meet the agency’s current productivity standard for the position. Participate in regular, clinical supervision with the Program Director and/or designee. Participate in professional trainings, center meetings, and staff development trainings as required or as appropriate. Guards client confidentiality by being knowledgeable of and adhering to HIPPA regulations. Adhere to all ACCESS policies and procedures. Maintain CANS/ANSA certification. Travel and driving company or personal vehicle as required for service delivery and professional development. Contribute to a positive, collaborative work environment. Cooperative, cordial and honest interactions with all employees are observed Report to work in a dependable manner, regularly and on-time. Request approval for scheduled leave in advance. Does not show evidence of excessive use of unscheduled leave or tardiness. Participate in coverage for team members as assigned. Performance metrics and job duties may change to reflect job updates and operational needs of the organization.

Job Type: Full-time

Pay: $21.26 - $23.43 per hour
