About the Role:
- Develop and implement evidence-based treatment plans for patients
- Collaborate with our multidisciplinary team to provide comprehensive patient care
- Assess patient health and identify areas for improvement
- Participate in quality improvement initiatives to enhance patient outcomes
- Contribute to the development and implementation of renal care programs
Requirements:
- Master's degree in Nursing and a graduate of an accredited Nurse Practitioner curriculum
- National certification as a nurse practitioner by one of the following certifying bodies: American Academy of Nurse Practitioners, American Nurses Credentialing Center, or The Certification Board for Urologic Nurses and Associates
- Licensure to practice in the State of Illinois; Controlled Substance License and DEA preferred
- One year of experience as a nurse practitioner is preferred
