Job Summary: Performs basic duties of patient care to adult and geriatric populations, under the guidance and direction of the Registered Nurse. Performs various housekeeping and cleaning duties to maintain a safe, clean environment. Performance expectations are defined in the competency evaluation.

Job Responsibilities:
- Admits/Discharges/Transfers patient and completes appropriate forms
- Provides for patient hygiene
- Provides for patients food and fluid needs
- Assists with patient mobility
- Vital signs
- Maintains safe environment of care
- Provides for appropriate pressure ulcer prevention
- Completes documentation accurately and in a timely manner according to hospital policies and procedures
- Provides age-appropriate and population specific patient care in a safe, skillful, accurate and efficient manner to primarily adult, and geriatrics populations as indicated
- Supports patients rights and responsibilities, including respect of cultural, ethical, and religious diversity.
- Adheres to standard precautions and infection control standards.
Education Requirements and Other Requirements:
Education Level: High school diploma or equivalent
Certification/Licensure: Successful completion of approved Nurse Assistant course with certification
by the State of Illinois
BLS certification required
Experience Requirements: Long term care, rehabilitation, and/or medical surgical care
experience preferred
Working Conditions: Exposure Category I: Tasks that involve exposure to blood, body fluids, or tissues. All procedures or other job-related tasks that involve an inherent potential for mucous membrane or skin contact with blood, body fluids, or tissues, or a potential for spills or splashes of them, care Category I tasks. Use of appropriate protective measures is required for every employee engaged in Category I tasks.
