About the Role
This position involves working closely with interdisciplinary teams to provide compassionate, high-quality care to individuals recovering from stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury.
Key Responsibilities:
- Assess patients' needs, developing and implementing individualized plans of care that prioritize their well-being and recovery goals.
- Liaise with multidisciplinary teams to ensure seamless coordination of care, communicate effectively with patients, families, and colleagues.
- Educate patients and their support networks on safe medication practices, proper wound care, and other essential topics related to their condition.
- Document patient interactions, treatment plans, and progress toward goals in a timely and accurate manner.
- Stay up-to-date with industry developments, best practices, and emerging research to enhance your professional growth and contribute to continuous quality improvement.
Qualifications:
- Recent LPN/LVN graduate or experienced practitioner with a strong commitment to patient-centered care.
- Strong communication and interpersonal skills, with ability to work collaboratively in a fast-paced environment.
- Ability to think critically and make sound judgments, prioritizing tasks and managing time effectively.
