Anderson Valley Health Center (AVHC) is located in beautiful Mendocino county known for its Wine, Waves and Wilderness, 2.5 hours north of San Francisco. Anderson Valley Health Center is a Federally Qualified Health Center and is the sole provider in the Anderson Valley area. We serve a unique population of general patients and migrant and seasonal farm workers. AVHC is currently seeking a Quality Improvement Coordinator to join our Population Health team and help provide excellent care to the residents of Anderson Valley.
Anderson Valley Health Center’s mission is to provide affordable high quality care to all valley residents. The clinic opened its doors in 1976 as a “free clinic” and continues today as an independent rural Federally Qualified Health Center providing “womb to tomb” care. Anderson Valley patients comprise of approximately 40% Hispanic/Latino, 20% Senior citizens, 45% Medi-Cal and 20% Sliding Fee Scale patients. In our single site, we operate services including; Medical, Dental, Behavioral Health, in-school nursing and health-education, an on-site medication dispensary and group support and health coaching for chronic disease patients. We see approximately 2,800 patients a year in 13,000 visits.

Summary:
Under the direction of the Population Health Director, the Quality Improvement Assistant supports AVHC’s QI Plan, Annual Work Plan, and Pay for Performance programs by conducting outreach and education to patients with care gaps, and other essential functions of the QI/PI Committee
Core Responsibilities:
- Support and monitor activities as outlined in AVHC’s Quality Improvement Plan and AVHC’s Annual Quality Improvement and Performance Improvement Work Plan
- Assist in the coordination, agenda and minutes for monthly QI/PI Committee meetings
- Perform outreach to patients with Care Gaps for Partnership QIP and AVHC QI Work Plan
- Coordinate AVHC’s Quality Improvement committee monthly meetings
- Use LEAN methodology to maintain visible data monitoring systems and documentation for identified QI projects and indicator monitoring
- Assist in the collection of data for identified LEAN process improvement projects and PDSA cycles
- Assist in providing support/resources (e.g. LEAN, QI, PCMH education and training) for all Clinic staff as requested
- Assist in the coordination Patient Satisfaction surveys, analysis, reporting and improvement projects.
- Assists in the performance of data validation and preparation of reports to meet various reporting requirements (UDS, PCMH,etc.)
- Assist in the preparation of summary reports for presentation to various committees including Quality Improvement/Performance Improvement, AVHC executive team and AVHC Board of Directors
- Provide administrative and special project support to the Executive team as needed
- Assist in supporting the Medical Director in providing leadership in the development and maintenance of AVHC’s QI Work Plan and Patient Centered Medical Home
- Work collaboratively with Administration to ensure the QI processes support AVHC Strategic Plan
- Work with Department Managers to facilitate various QI Committee meetings and support data driven decision making and implementation of new processes
- Additional duties as assigned
Minimum Requirements:
- Preferred minimum 2 years experience in a health care setting with knowledge of population health.
- Proficiency in software programs utilized in Quality Improvement and/or EMR report generation tools
- Excellent skills in MS Office programs
- Bilingual in English and Spanish
Nice to have:
- Preferred two year college degree or equivalent experience in the health care field.
- Experience with Electronic Health Records
