Must have Bachelors degree in human service field.
The main purpose of a Service Coordinator is to support and assist people with intellectual disabilities in achieving their personal goals. The service coordinator assists consumers in the 5-county service area in accessing medical, social, educational and other appropriate services and supports that will help that individual achieve a quality of life and community participation that is acceptable to the individual. This is accomplished through: 1) assessment, 2) service planning and coordination, 3) monitoring, and 4) crisis prevention and management.
Meets with consumer/LAR to identify personal goals/preferred outcomes. Identifies existing and needed services and supports. Completes Service Coordination Assessment. Performs uniform assessment. Facilitates person directed planning process. Develops plan of services and supports. Works with Center program and other agencies for the delivery of services. Review PDP quarterly. Monitors to ensure that clients receive needed services. Works to prevent or manage crisis. Documents all the above in client record. Bills for services.
