Home Care Team Coordinator, Lewiston / Norway
What Youll Do :
- Staff Home Care clients with Personal Support Specialists (PSS), Certified Nursing Assistants (CNA), and Caring Companions (CC).
- Enter visit frequency orders for Home Care services into EMR system.
- Confirmation of weekly schedules with staff and clients.
- Collaborate with leaders and other members of the Home Care team to increase the patients served in various regions of Maine.
- Collaborate with Human Resources to recruit and maintain a growing team of extraordinary direct care workers.
