Title : Physical Therapist Weekend Baylor, Farmington
What Youll Do :
- Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association. Services may include screening, evaluation, direct service, consultation and / or instruction.
- Demonstrate knowledge and case management skills to coordinate client home care needs in collaboration with client, family, physician, other providers and pay sources towards outcomes promoting client independence and well-being.
- Develop, implement, coordinate, and revise a plan of care that incorporates goals consistent with the clients age, condition, and needs.
- Implement client care plan activities based on treatment goals towards independent living for the client / family.
- Work 30 hours per week, get paid for 36 hours per week!
