What are the responsibilities and job description for the Provider Relations Consultant position at Androscoggin Home Healthcare + Hospice?
Provider Relations Consultant - Presque Isle
You will:
- Develop business partnerships and relationships with primary and specialty care practice leaders, medical staff and administrative teams.
- Grow referral volume for all Andwell Health Partner service lines.
- Initiate the referral-to-admission process for each patient referred.
- Update and maintain customer and competitor profiles, log daily call activity, and document plans for follow-up activities with referral sources.
You bring:
- In-depth knowledge of Andwell's Home Health, Home Care, Hospice and Palliative Care programs, as well as Mobile Wound Care, Outpatient Therapy, Community Care Team and Behavioral Health.
- One year of recent clinical nursing experience.
- At least three years of experience in direct sales, case management or administrative nursing role, with a proven record of achieving incremental growth.
